Information for Patients

To make an appointment

Please telephone the Practice to make your appointment on 01656 766866. Our phone lines are open from 8.00 a.m. to 12.00 p.m. and from 1.30 p.m. to 6.30 p.m.
Alternatively use our Econsult service to request advice or treatment by the end of the next working day.

When you request an appointment, we will ask the reason for the appointment and will direct your medical problem to the most appropriate service or healthcare professional who can help or support you.

You may request an appointment with a specific doctor and we will do our best to accommodate you, subject to availability.

We offer a mix of appointments- some face to face and some telephone consultations. We have appointments available on the same day and also some that can be booked in advance.

If you are unable to use a telephone or if you do not have access to one, please call into the practice to inform the receptionist who will assist you and ensure your records are updated.

If you are unable to keep an appointment, please ring the cancellation line open 24hrs: 01656 673355. This is most helpful as the free appointment could be taken by someone else

Urgent Appointments

If you feel that your problem is so urgent that it needs to be seen the same day, please inform the receptionist, who is here to help you. Urgent problems will be dealt with on the same day or immediately if necessary.

Housecalls

To request a housecall please telephone the surgery between 8.00 am – 11.00 am. Our doctors will only visit a patient at home when the patient is unable to attend the surgery due to illness or disability. We always encourage patients to come into the surgery because this is where the care can be provided. The doctors only have access to your full medical records at the surgery. Specialist equipment is here, examinations and tests can be carried out more easily and some conditions can only be treated at the surgery.

Repeat Prescriptions

Please order your repeat medication by dropping your repeat slip into a community pharmacy or ringing the prescribing hub Mon to Fri 9am -4pm: 01656 311010, or via My Health Online (see our website for details) or post (by hand) your repeat slip through our letter box, marking which medication you require.

Please assign a pharmacy to your prescriptions so that you do not have to attend the surgery to pick them up.

Out Of Hours

The Practice is open Monday to Friday from 8.00am - 6.30pm every day except Bank Holidays. Outside normal surgery hours there is a recorded message on the telephone - this message gives details of the out-of-hours cover. There is a telephone number for NHS Direct 111.

Out-of-hours services are the non-emergancy health services provided when your GP surgery is closed in the evening, at night and throughout the weekends. They are for patients who ned to see a doctor urgently and who cannot wait until the next day.

Emergencies

Urgent problems will be dealt with on the same day or immediately if necessary.

Please attend the surgery whenever possible. The doctors are able to assess you better if they have access to your notes, computer record, and the facilities present in the surgery. We try to reserve house calls for patients who are too unwell to travel to the surgery.

PATIENT INFORMATION

  • Our receptionists will ask you the reason for your appointment with the doctor so that we can help you receive the appropriate level of care and direct you to the correct member of the Team to deal with your health needs. Please be aware that reception staff, like all members of the team are bound by confidentiality rules. Any information given by you is treated in the strictest confidence. Your co-operation would be greatly appreciated. Please help us to help you.

    Please ring 01656 766866 for an appointment or use our econsult service. Our phone lines are open at 8am and close at 6:30pm. You may ask for the doctor of your choice and we will try to accommodate that, subject to the doctor's availability on that day. A GP or Nurse Practitioner will ring you back and speak to you and decide whether you need to come to the surgery to be assessed. Please ensure that you have an up to date contact number and will be able to answer the phone when we ring. We also offer appointments with Pharmacists, Physiotherapist and Nurses which can be pre-booked. Many of our clinic appointments are face to face. If you come to the Practice for an appointment we require you to wear a face covering please.

    Please limit your complaints. Please try not to bring “lists of problems” to your appointment, it is very difficult for the doctor to deal with a number of problems effectively in a single appointment. It also is a major cause of long waiting times for the other patients.

    Cancelling Appointments

    We have a 24 hour cancellation line. The telephone number is 673355. It would be a big help to the Practice if you could ring this number and leave the details if you wish to cancel an appointment with the doctor or the nurse. We can then offer these appointments to other patients. (Did you know we average more than 200 non-attendances a month. That is the same as 10 surgeries - per month!!)
  • How to Register:

    1.Bring your medical card (and those of any member of your household who wish to register) to the reception area in the practice. If you have mislaid your card(s) the receptionist will give you forms to complete.

    2. As part of your registration, you and any household member over 15 years old, will be asked to complete a New Patient Registration Form and to take your blood pressure on the machine available in reception. Our receptionists will be pleased to give you instruction on this.

    3. It is very important to sign all your forms as it will delay your registration if they are not signed. We also require details of any younger children's immunisations when you register so that we can continue with their immunization program.

  • Clinical Pharmacist: We now offer patients the option of either a face to face appointment or a telephone consultation with a clinical pharmacist. This is often more convenient than making an appointment with the GP. Download the leaflet here

  • Our doctors will always encourage patients to come into the surgery because this is where the best care can be provided.

    The doctors only have access to your full medical records at the surgery. Specialist equipment is here, examinations and tests can be carried out more easily, and some conditions can only be treated at the surgery.

    Our doctors will only visit a patient at home when the patient is unable to attend the surgery due to illness or disability.

    In the time it takes to visit one person at home the doctor can see three or four patients at the surgery.

    For these reasons, if you request a home visit, the doctor may call you back to find out more about your problem before deciding whether to come out.

    Please help us to help you, by coming to the surgery to see the doctor if you are able to do so.

  • If we have your current mobile telephone number, we will send you appointment reminder text notifications, invitations to clinics and health promotion information.

    Please ensure that we have your most up to date mobile telephone number so that you can benefit from this service, or for this service to continue if you are already receiving texts from us. (You can opt out of the text notification service at any time by contacting the practice).

  • Riversdale Charitable Trust began with approaches by patients who wished to make gifts or bequests for the benefit of other patients. It is now a registered charity which has been set up to raise funds to purchase a variety of materials and equipment. These will be used for the care of patients, especially in areas where the provision by the NHS could be even better.

    The Trust is Administered by a Board of Trustees.

    The sort of equipment we have purchased in the past include syringe drivers, spenco mattresses, a suction pump, resuscitation equipment, ECG machines, Doppler equipment, ambulatory blood pressure monitors, a wheelchair,a phlebotomy chair and our new airconditioning system.

    The majority of the equipment is used by our practice nurses, community nurses and others who are concerned with the care of our patients, often at home. Such equipment improves the quality of our patient's lives.

    How Can You Help?

    Some of the ways you can help include:

    • Donations in memory of a loved one
    • Covenants
    • A bequest in your will
    • Request for “donations” in lieu of flowers in funeral notices
    • Donations for a specific purpose

    We ask you to treat our Doctors and all practice staff with courtesy and respect.

    The first hour of the morning can be extremely busy. Please keep telephone calls brief. If possible,leave routine calls until later in the day.

    You are responsible for your own health and that of your children. Please take the advice given to you at the practice. Let us know immediately if you change your address or name. Remember to give your phone number and postcode.

    Please contact the surgery between 8.30 and 11am for a home visit during the day.

    If tests are ordered for you , please ask your doctor or a member of staff about receiving the results.

  • Please advise reception if you change your name, address or telephone number . It would be a great help if you would also ensure that we have your correct telephone number and your postcode. Please remember to also inform the hospital if you are currently under their care or waiting for an appointment. Please fill out a form in reception to update your information, it is important we hold current information about you.

  • We are privileged to be a training practice. This means that doctors training in general practice work in our practice as part of a three year programme involving experience both in general practice and in various hospital specialties. They are with us for six or twelve months and will have acquired considerable hospital experience before joining us. Trainees sometimes sit in with the GPs in their surgeries. Please consider them as you would a partner.

    Registrars who come to the Practice will receive a Welcome Pack containing information about the surgery and how we work.
