During the hours the surgery is closed if you require advice about a medical problem you can call NHS Direct on 0845 46 47. If you need a doctor in an emergency you can call: 01656 860343 where you will be given details of who to contact.

  • Appointments can either be arranged by telephone or through MY HEALTH ON LINE. To book an appointment via my health on line you will need to register on the website.     During the Covid-19 pandemic we amended our procedure for booking Doctors’ appointments we are now in the process of reintroducing some prebook able and Face to Face appointments.

     

    The Doctors have asked members of the Care navigation team to ask a few questions to ensure that the correct health professional is referred to. It is still the Norm for the first contact with a Doctor to be via a telephone call back, when the Doctor will discuss your medical problem. Following this discussion the Doctor may ask you to visit the practice for a face 2 face consultation or refer you to the appropriate Care Team.

  • Patients can now book appointments and request repeat prescriptions online. The service, known as My Health Online, is part of a Welsh Government initiative to improve patient care through better use of IT and information. It is brought to you by corroboration between your GP practice and the NHS Wales Informatics Service. To use these facilities you first need to register with the practice.

    Please complete the More information on the following document… Registration form form and return with identification (preferably a driver licence or passport, or other documents confirming your name, date of birth and address) and the receptionist will take the details from you. We aim to return the PIN numbers to enable you to register online within 20 working days.

    Your personal details are protected using the highest standards of Internet security to prevent them being intercepted. It is your responsibility to keep your log in details safe.

    Booking an appointment online

    Using the Internet site My Health On line you are able to book a selection of available appointments. You will get confirmation back immediately online, so there is no need to re-confirm by phone or email. Just arrive promptly on the day. You can also view any appointments you have already made or cancel them if necessary.

    Requesting a repeat prescription

    Using the Internet, you don’t have to visit the practice to hand over your repeat prescription request. Once requested simply collect your prescription as usual. If you already have arrangements in place for us to forward your prescription to particular pharmacy we will continue to sent them to your pharmacy of choice.

    Information regarding My Health On Line

  • A form/questionnaire is avaiable from our care navigator staff who will provide an appointment for a registration check up.

    Patients can express a preference to receive services from a particular doctor. Further information can be obtained from our care navigators.

  • If you need medical advice NHS direct can be contacted on 0845 46 47. However if you need to speak to a Doctor we can arrange for a Doctor to telephone you. Please telephone after 9.00am if you wish to speak to the Doctor over the phone and a message will be taken and provided to the Doctor.

    Please telephone for any test results after 1.30pm.

  • If you are too ill to attend the surgery and feel you require a home visit please telephone before 10.00am. Our care navigators will require certain information in order to enable the doctor to allocate priority to house calls. Please note that a lack of transport is not a criteria for a home visit.

  • In addition to the standard general medical services, the practice also provides:

    Health Checks:

    • Patients who are aged between 16-75 who have not been seen within the last 3 years can attend the surgery for a routine check up – see above regarding making an appointment.
    • Patients over 75 who have not been seen in the last year can attend the surgery for a routine health check – – see above regarding making an appointment.
  • Patient Responsibilities:

    • A prescription is not always necessary with every consultation.
    • Home visits are intended for those patients who are too ill to attend surgery.
    • Please be patient with our care navigators, they undertake to do as much as possible to meet your needs.
    • If you are unable to attend an appointment please telephone to cancel.

    Our Patients Charter:

    • You will be treated in a courteous and friendly manner by all our staff.
    • Your records will be treated confidentially at all times.
    • We promote a healthy lifestyle and will offer free health promotion services.
    • All parents are offered immunisation services for their children.
    • All patients over 75 years of age can take advantage of annual health checks.
    • Cervical smears are recommended every 3 years; we will send a reminder to all eligible patients.
    • We will provide training to staff to improve our services to you.
  • If you change your name, address or telephone numbers please can you advise us in writing. A form is available from our care navigator staff for this purpose.

    Please note that it is important that we have correct up to date contact details in case one of the primary care team needs to contact you.

    The new 111 service has been introduced in this area and has replaced NHS Direct.

    For out of Hours contact phone 111 This number is available on the answerphone message by calling the surgery telephone number out of hours.

    Please note that the Abertawe Morgannwg University Local Health Board is responsible for the commissioning of the out of hours services.

    A summary of the 111 Wales service:

    111 is the new, completely free way to contact the NHS from landlines and mobiles. The new service is being piloted in the Abertawe Bro Morgannwg University Health Board (ABMU) area, which means you can currently only use the service if you are registered with a GP practice in Swansea, Neath Port Talbot or Bridgend.

    The service is an amalgamation of NHS Direct Wales and the GP out-of-hours service and is currently being piloted in Swansea, Neath Port Talbot and Bridgend. If you live outside these areas or are registered with a GP practice in another county, you won’t be able to ring 111.

    The 111 service is available 24 hours a day, seven days a week, and you can use it both for health information and advice and to access urgent care.

    How it Works

    Sometimes it’s difficult to know which service to contact and when, so introducing this memorable three-digit number will make it easier not just to access health information and advice if you are ill are unsure what to do, but to access urgent care once your GP surgery has closed.
